PROBATE ADDENDUM: Affidavit of Creditor

This affidavit is an attachment that can be submitted along with the probate form petitioning for transfer of a deceased person's personal property. ONLY include this with your petition if you are a creditor with a right to the decedent's property as opposed to a spouse, designee, or next-of-kin.
